Files, notes, and records pertaining to individuals served by AZ Benefits Connection, LLC are confidential. These documents are only shared on a need to know basis with authorized agencies and companies, such as a government regulator or insurance company and/or their General Agency.

The Information I Collect:

This notice applies to all information submitted through direct secure links to insurers found on Helpyourselfinsurance.com

I do not collect any application information directly on Helpyourselfinsurance.com

On the Individual and Family page you may enroll in an ACA/Obamacare plan, or apply for other non ACA health insurance products. The same may apply when applying for Dental coverage. The types of personal information collected at these secure pages are:

Health History for underwriting. (Non ACA plans only)

Phone number
Credit/Debit Card Information
(etc.)

On the Medicare Page there are no enrollment portals, nor requests for any (PII) Personally Identifiable Information, or (PHI) Personal Health Information.

Any information collected will be provided by you to me on an (CMS) Centers for Medicare & Medicaid Services approved application created by and submitted directly to the insurance company issuing coverage.

By providing me with the above information, you consent to my using it to secure new coverage for you.

PREMIUM PAYMENT

All premiums due to an insurance company will be billed by them and paid directly to them by you. At no time will I collect money directly from you on their behalf.

PROFESSIONAL FEES

I am compensated directly by the insurance company issuing your coverage for assisting you.

However, in the event of an (ACA) "Obamacare" plan not working with/paying brokers, I may charge you a fee for my time and guidance at your request. You understand and agree that I am only assisting you with making an informed decision, and will not be considered Broker of Record by the insurance company.

At no time will I ever charge any fee in relation to assisting you with Medicare coverage.

The Way I Use Information:

I use the information you provide about yourself and any dependents to find suitable coverage for you and/or your family. I do not share this information with outside parties except to the extent necessary to get your coverage issued.

Finally, I never use or share the personally identifiable information provided to me online in ways unrelated to the ones described above without also providing you an opportunity to opt-out or otherwise prohibit such unrelated uses.

My Commitment To Data Security

To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, I have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information transmitted to an insurer on its secure portal.
